What a designated caregiver can do
Within New York's medical program, a certified patient can designate a caregiver who registers with the state and can then purchase medical program products for the patient at registered medical dispensaries. The program defines who can serve as a caregiver, how registration works, and what caregivers can buy. Those rules live with the Office of Cannabis Management at cannabis.ny.gov, and checking them directly is always the right move because program details change.
The specifics OCM publishes, read on August 9, 2026: a patient may designate up to five caregivers, and they may be added any time after the patient is auto-registered. The patient designates first, then the caregiver registers separately.
At the counter, a caregiver purchasing on a patient's behalf presents the patient's certification, their own caregiver registry ID, and a government-issued photo ID. There is no physical registry card to produce, because OCM states registry ID cards are no longer being issued.
Patients under 18, and patients unable to consent
This case is not optional. OCM states that patients under the age of 18, or incapable of consent, are required to have one designated caregiver, who is auto-registered at the time of certification by the certifying health care provider.
If that is your household, confirm it is being handled during the certification appointment rather than discovering afterwards that it was not. The caregiver brings the patient's certification, the caregiver registry ID, and a government-issued photo ID.

Facilities as caregivers
New York also lets certain facilities register as designated caregivers. OCM lists eligible facility types under the MRTA, including hospitals, adult care facilities, schools, and research institutions with an institutional review board, and states there is no limit on the number of patients a designated caregiver facility may serve.
It is not mandatory for a facility to become a designated caregiver, so a facility declining is a policy choice rather than a legal barrier. To purchase at facility level, a copy of the patient's registry ID and the approved facility caregiver form are presented to the registered organization.
Any facility employee the facility deems appropriate may administer product, and patients may self-administer where they are capable and facility policy allows.
What caregiving does not cover at an adult use store
